29 CFR 1910.147(a)(3)(i):The employer did not utilize procedures for affixing appropriate lockout devices or tagout devices to energy isolating devices, and to otherwise disable machines or equipment to prevent unexpected energization, start up or release of stored energy in order to prevent injury to employees: a. On or about 05/28/2021, at pack line 16 - employees were exposed to caught-in or crush by hazards, in that, the employees were allowed to introduce their hands between the moving rollers to remove garment from the powered conveyor rollers without using lockout\tagout procedures.

29 CFR 1910.219(f)(3): Sprocket wheels and chains which were seven (7) feet or less above floors or platforms were not enclosed: a. On or about 06/09/2021, at the packing area conveyors - employees were exposed to caught-in or crush by hazards, in that, the mechanical power-transmission chain and sprocket guards were broke, some were fixed with duct tape and the duct tape was also damaged, creating large opening. Additionally, chain and sprocket guards was not secured, in that, bolts or fasteners were not installed.
