On or about 06/09/2021, at the packing area conveyors - employees were exposed to caught-in or crush by hazards, in that, the mechanical power-transmission chain and sprocket guards were broke, some were fixed with duct tape and the duct tape was also damaged, creating large opening. Additionally, chain and sprocket guards was not secured, in that, bolts or fasteners were not installed.
