29 CFR  1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about May 28, 2021, the employer did not ensure energy control procedures were utilized when an employee was clearing a jam on the Line 16, Alloyd blister pack machine (Model: 18SC1216, Ser#: 97605).

29 CFR  1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  On or about May 28, 2021, the employer did not ensure energy control procedures were utilized when an employee was clearing a jam on the Line 16, Alloyd blister pack machine (Model: 18SC1216, Ser#: 97605.  The employer failed to implement energy control application steps as the machine was not shut down or turned off to perform the servicing work  [per the 1910.147(d)(2) requirements].  As a result, the remaining applicable energy control elements, involving machine isolation [(d)(3)], LOTO device application [(d)(4)], dissipation of residual energy [(d)(5)(i)], and verification of isolation [(d)(6)], were not implemented to protect employees from machine servicing hazards.

29 CFR  1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed:  On or about June 9, 2021, the employer did not ensure periodic inspections of energy control procedures were conducted annually, including a review of each authorized employee's responsibilities under the energy control procedures.  Authorized employees are required to conduct and direct service/maintenance activities on the  Alloyd Blister Pack Machine (Model 10SC1216) requiring them to perform lockout/tagout.

29 CFR  1910.147(c)(7)(i): The employer shall provide training to ensure that the purpose and function of the energy control program are understood by employees and that the knowledge and skills required for the safe application, usage, and removal of the energy controls are acquired by employees. The training shall include the following:  On or about May 28, 2021, the employer failed to provide training to ensure affected and authorized employees understood the purpose and function of the energy control program and employees had acquired the knowledge and skills required for the safe application, usage, and removal of energy controls.  The employer engages in the service and maintenance activities on machinery and equipment including, but not limited to; the Alloyd Blister Pack Machine, Model 10SC1216.
