A-frame ladder to access the working platform of a mobile scaffold and the ladder did not extend to the working platform landing.

29 CFR 1926.454(a): The employer did not have each employee who performed work on a scaffold be trained by a person qualified in the subject matter to recognize the hazards associated with the type of scaffold being used, and to understand the procedures to control or minimize those hazards. Location: 1833 Nostrand Ave. On or about: 9June 2021 a) the employer did not ensure employees working from a mobile scaffold were trained by a qualified person. Employees worked on platforms not fully planked, accessed scaffold landings with an A-frame ladder that was not long enough to reach the working platform and did not wear their personal fall arrest equipment correctly.
