Located at the construction site along Penny Way in Delaware, Ohio: On or about June 9, 2021, employees used a piece of two inch metal angle as a spreader beam to hoist a sign on a gas station structure. The employer did not ensure that the custom lifting device was tested and labeled with the safe working load.

Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. Located at the construction site along Penny Way in Delaware, Ohio: On or about June 9, 2021, an employee was installing a sign from the roof of the gas station structure. The employee was not protected from a fall from the roof's edge, thereby exposed to an approximately twenty foot fall hazard.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. a. Located at the construction site along Penny Way in Delaware, Ohio: On or about June 9, 2021, the employer did not ensure that the employee installing a sign from the roof of the gas station structure and from a man basket was trained on the recognition of fall hazards and fall protection systems needed to protect from the hazards.
