On or about June 6, 2021, on a residential construction site located on 2827 Merlin Way in Katy, TX, an employee was exposed to a fall hazard greater than 6 feet while performing masonry work on the roof of a two-story house without the use a fall protection system.

On or about June 10, 2021, on a residential construction site located on 2827 Merlin Way in Katy, TX, an employee was exposed to a fall hazard when trying to access the roof of the residential home without proper access.
