29 CFR 1910.263(i)(11)(i): All top openings should be provided with covers attached to the machines. These covers should be so arranged and interlocked that power will be shut off whenever the cover is opened to a point where the operator's fingers might come in contact with the beaters. On or about June 10, 2021, employees were operating a Hobart vertical commercial mixer (Serial No. 1948365 / Model #H600 60 qt. - 115V, 3 Phase, 1 HP) with the top opening left unguarded. This condition exposed employees to caught-between hazards.
