29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical agents: On or about June 8, at 1230 PM on the east side of the site employees were using a STIHL Chop saw to cut a concrete slab without eye or face protection.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 5, 2021 east side of the site construction site, employees were performing remodeling work of a parking garage on the third and fourth level of the unprotected edge without fall protection at heights upwards of twenty (20) feet.
