The written program shall be available for inspection by employees and their authorized representatives. (a )AAA Concrete Products, Inc., at 12672 WCR 6 1/4, Ft. Lupton, CO 80621: On and before June 9th, 2021, the employer did not ensure that employees were protected from permit-required confined space hazards, in that employees had been entering a structure via a top hatch to break up and chisel solidified concrete without a written permit space entry program in place. This condition exposed employees to entrapment hazards.

29 CFR 1910.146(g)(1): The employer did not ensure that employees were provided the proper training in order to acquire the understanding, knowledge, and skills necessary for the safe performance of the duties assigned in permit-required confined spaces. (a) AAA Concrete Products, Inc., at 12672 WCR 6 1/4, Ft. Lupton, CO 80621: On and before June 9th, 2021, the employer did ensure that employees were protected from permit-required confined space hazards, in that employees entering a structure via a top hatch to break up and chisel solidified concrete were not provided the proper training to perform their duties safely. This condition exposed employees to entrapment hazards.
