29 CFR 1926.451(e)(4)(iv): Stair rail systems and handrails shall be surfaced to prevent injury to employees from punctures or lacerations, and to prevent snagging of clothing. On or about June 10 and June 15, 2021 and times prior, employees including the manager were using a stair tower to access the roof deck and the bolts in use to attach the handrails projected out into the walking space to the stair rail system.

29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about June 10 and June 15, 2021 and at times prior, employees were working from the roof deck without the use of any fall protection system.
