29 CFR 1910.178(p)(1): If at any time a powered industrial truck is found to be in need of repair, defective, or in any way unsafe, the truck shall be taken out of service until it has been restored to safe operating condition: Building 1: Employer is exposing employees to hazardous conditions where the employees are permitted to operate forklifts which have defects. On and about 6/10/2021 employees operated forklifts which had defects such as, but not limited to, non-functioning brake lights, exposing the employees to struck-by hazards. The Nissan forklift was not removed from service until the defects were corrected.

About 06/10/2021 employees operated a Craftsman 12 inch vertical band saw which did not have a guard on the rotating edges of the blade exposing the employees to amputation hazards.

On and about 6/10/2021 employees operated Commercial Electric pedestal fan which had openings in the guard which were measured at approximately 1 inch and 1 1/2 inches exposing the employees to amputations hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure: Establishment: Employer is exposing employees to electrical hazards where the employees are permitted to operate a piece of fixed electrical equipment powered by a flexible cord. On 6/10/2021 a flexible cord was used to supply power to a piece of fixed die storage equipment. The power cord of the fixed equipment was connected to the flexible cord using the flexible cord as a substitute for fixed wiring of a structure. Employees who operated the Kardex Remstar Shuttle XP 700 equipment were exposed to electrical hazards.
