29 CFR 1910.212(a)(1): Machines were not guarded in order to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:   On or about 06/07/2021, employees were operating paper bag machines without guards in place exposing the employees to rotating parts:  a)  Potdevin paper bag machine (B1-Production line)  guard open not in place while employees were operating the machine. b)  Potdevin paper bag machine (S2-Production line) back side guards missing exposing employees (S1-Production line) to rotating parts. c)   Potdevin paper bag machine (S5-Production line) guards not in place.

29 CFR  1910.37(b)(6):Each exit sign was not illuminated to a surface value of at least five foot-candles (54 lux) by a reliable light source and be distinctive in color:  On or about 06/07/2021, Employees were working at the facility without illuminated exit sign doors to exit to the outside of their work areas in case of an emergency:  a) Exit signs were not illuminated over exit doors in the administration office area.   b) Exit signs were not illuminated over exit doors in the paper manufacturing shop area.
