Eye protection was not being utilized while employees engaged in this activity.

29 CFR 1926.503(a)(1): The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards: On or about June 8, 2021, at jobsite located in 230 Michigan St., Elmhurst, IL 60126, the employee failed to train employees on the recognition of fall hazards and the means and methods to minimize these hazards. Employees were performing roofing work at a new residential property and were exposed to fall hazards in excess of 20 feet when fall protection was not being used.
