Defective rigging equipment was not removed from service. a.) 661 Main Street, Holden, MA: On or about June 11, 2021 and at times prior employees were exposed to struck-by and crushing hazards while lifting a road plate with a defective lifting chain. The employer did not inspect and the employer did not remove defective equipment from service prior to use. The chain did not have a permanently affixed and legible identification marking as prescribed by the manufacturer that indicate the recommended safe working load and the load pin on hook had been replaced with nut and bolt. b.) 661 Main Street, Holden, MA: On or about June 11, 2021 and at times prior employees were exposed to struck-by and crushing hazards while lifting a second road plate with a defective lifting chain. The employer did not inspect and the employer did not remove defective equipment from service prior to use. The chain did not have a permanently affixed and legible identification marking as prescribed by the manufacturer that indicate the recommended safe working load and the hook had been removed. c.) 661 Main Street, Holden, MA: On or about June 11, 2021 and at times prior employees were exposed to struck-by and crushing hazards while lifting a trench box with chains that had no permanently affixed and legible identification markings as prescribed by the manufacturer that indicate the recommended safe working load.
