Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to fire hazards: a) A2B Solutions Inc. at 9255 Pierce Street in Westminster, CO: On or about 6/2/2021, an employee used a Mr. Heater Gas Fired Infra-Red Tank Top Propane Heater, model MH 540ET, improperly. The employer did not ensure the equipment used to conduct bed bug mitigation was safe for the work setting. The employer did not follow the manufacturer's instructions when operating the Gas Fired Infra-Red Tank Top Heater. Due to its improper use, the heater and propane tank on which it was set tipped over, which ignited a fire that severely burned the employee.

Injury occurred on 6/2/201 and was reported to OSHA on 6/10/2021.
