29 CFR 1910.147(f)(1):When lockout or tagout devices temporarily were removed from the energy isolating device and the machine or equipment was energized to test or position the machine, equipment or component thereof, the sequence of actions were not followed. On or about June 9, 2021 an employee was exposed to amputation hazards while performing set-up operations on a "Proto" bead roller the employer did not ensure that the employee followed that required sequence of actions, in that the guard was not replaced and the employee was not removed from the machine prior to energizing the machine and testing the equipment.
