29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  Cherry Point MCAS, Commissary: On or about June 14, 2021, where the employer maintains written hazard communication program on the hazards associated with use of chemical components such as, but not limited to D-Glucopyranose, oligomeric, decyl octyl glycosides (Skillcraft All Purpose Cleaner), 1.0%, and Sodium Hypochlorite (Bleach), 6%-6.3%, the plan is not updated to the 2012 HazCom update and contained the following deficiencies:  1. Methods used to explain and train employees on the the specific criteria for classification of health and physical hazards, as well as classification of mixtures, pictograms, new labels elements and safety data sheets new specified 16-section format to facilitate recognition and understanding.  2. Methods the employer will use to inform employees of the hazards in non-routine tasks, and the hazards associated with chemicals contained in unlabeled pipes in their work areas.  3. Describe how the training is to be conducted (e.g., written, visual presentation using slides, verbal). Procedure to train new employees at the time of their initial assignment and to train employees when a new hazard is introduced into the workplace. Procedure to train employees when they are potentially exposed to chemicals used by other employers, multi-employer worksites.  4. Describe how the data sheets are to be maintained (e.g., in notebooks in the work area(s), in a pick-up truck at the jobsite, via telefax), procedures on how to retrieve SDSs electronically, including backup systems to be used in the event of failure of the electronic equipment, and how employees obtain access to the SDSs. Procedure to follow if the SDS is not received at the time of the first shipment. Procedure to follow if it is suspected that the SDS is not appropriate (e.g., missing hazards). 29 CFR  1910.1200(h)(1):  Employers shall provide employees with effective information and training on hazardous chemicals in their work area at the time of their initial assignment, and whenever a new chemical hazard the employees have not previously been trained about is introduced into their work area. Information and training may be designed to cover categories of hazards (e.g., flammability, carcinogenicity) or specific chemicals. Chemical-specific information must always be available through labels and safety data sheets.  Cherry Point Commissary: On or about June 14, 2021,  the employer did not train employees on the hazards associated with mold and cleaning chemical exposure.   ABATEMENT CERTIFICATION REQUIRED

29 CFR  1904.5(a):Basic requirement. You must consider an injury or illness to be work-related if an event or exposure in the work environment either caused or contributed to the resulting condition or significantly aggravated a pre-existing injury or illness. Work-relatedness is presumed for injuries and illnesses resulting from events or exposures occurring in the work environment, unless an exception in �� 1904.5(b)(2) specifically applies.  Cherry Point Commissary:  On or about June 14, 2021, where an OSHA form 301 (First Report of Injury) was completed for an employee who suffered a recordable illness back in 2019, the employer did not ensure that work related lost time illness was recorded onto OSHA form 300 log for the 2019 calendar.   ABATEMENT DOCUMENTATION REQUIRED
