29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. On June 9, 2021 at 19 Main Street in Montello, Wisconsin, an employee was spraying water on demolition debris during removal activity with an excavator and was working from a debris trailer ladder without utilizing eye protection. The employee was guiding the loading of debris in the trailer and using water to control dust.

General-duty citation text
29 CFR 1926.850(a): Prior to permitting employees to start demolition operations, an engineering survey shall be made by a competent person of the structure to determine the condition of the framing, floors, and walls, and possibility of unplanned collapse of any portion of the structure. Any adjacent structure where employees may be exposed shall also be similarly checked. The employer shall have in writing evidence that such a survey has been performed. On or about June 9, 2021 at 19 Main street in Montello, Wisconsin, the employer did not perform or document an engineering survey prior to beginning demolition work on a condemned, two-story building built circa 1895. Employees were exposed to potential struck-by hazards while demolishing the building with the use of an aerial lift, hand tools, and excavators.
