29 CFR 1926.21(b)(2):The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about 6/2/2021 the employer did not ensure that employees were trained to understand the hazards associated with activities such as but not limited too; demolition of structures and gut rehabbing a facility in that employees cutting on an old paint booth were not trained on the control or elimination for fire suppression during the cutting.

29 CFR 1926.501(b)(1):"Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or before 6/2/2021 the employer did not provide fall protection for employees who were throwing debris into a hole from the second level, exposing employee to a 15' fall hazard.

29 CFR 1926.503(a)(1):The employer shall provide a training program for each employee who might be exposed to fall hazards. The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards. On or before 6/23/2021, employees were exposed to a fall hazard of approximately 15' while throwing debris into holes that had an approximate measurement of 20' x 12' cut into the concrete floor of the 2nd story of a building and had not received any training on recognizing the hazard of working at open holes and what fall protection to utilize to prevent employees from falling into the holes.

29 CFR 1926.859(g):During demolition, continuing inspections by a competent person shall be made as the work progresses to detect hazards resulting from weakened or deteriorated floors, or walls, or loosened material. No employee shall be permitted to work where such hazards exist until they are corrected by shoring, bracing, or other effective means. a) On or before 6/2/2021 the employer did not conduct inspection in order to detect hazards associated with conducting a gut rehab of a building, such as but not limited to; the hazards associated with cutting stainless that can cause a collapse of the equipment and fall hazards associated with large opening from the 2nd floor.
