All Phase Concrete Construction, Inc., at 2797-2799 W 13th Ave., Denver CO 80204: On or before June 15, 2021, the employer did ensure employees were protected from injury in that two employee was observed conducting formwork at heights of 15 feet without the use of a fall protection system. This condition exposed employees to broken bones.

All Phase Concrete Construction, Inc. at 2797-2799 W 13th Ave., Denver CO 80204: On or before June 15, 2021, as the exposing employer, did not ensure its employees and employees of I-kota LLC and Rockies Rebar Reinforcement LLC, subcontractors were protected from injury in that employees worked and walked around unguarded reinforcing steel protruding from concrete while performing work operations. This condition exposed employees to impalements.

All Phase Concrete Construction, Inc., at 2797-2799 W 13th Ave., Denver CO 80204: On or before June 15, 2021, the employer did not ensure employees were protected from injury in that one employee was observed operating a vibrator while standing on a step ladder positioned on two adjacent platforms thereby spanning a gap and which could be displaced by the workplace activities. This condition exposed employees to broken bones and head contusions.
