29 CFR  1910.28(b)(11)(ii):29 CFR 1910.28(b)(11)(ii): The employer did not ensure that each flight of stairs having at least 3 treads and at least 4 risers is equipped with stair rail systems and handrails as described in 29 CFR 1910.28 Table D-2-Stairway Handrail Requirements:  218 W. Main Street, Jackson, MO 63755, Hammer Building: Employer is exposing employees to hazardous conditions where the employees are permitted to use a flight of stairs which does not have a standard guardrail or hand rail system.   On and about 6/13/2021 employees used a flight of stairs 41 inches wide which had 5 risers to access the building.  The employees were exposed to fall hazard where a mid-rail was missing from the flight of stairs.

Main Street, Jackson MO 63755, Hammer Building: Employer is exposing employees to hazardous conditions where the employees are permitted to perform repairs and maintenance on a piece of equipment without specific lockout/tagout procedures.  Employees were exposed to amputation hazards where they performed lockout and tag out activities on the wooden handle inserting machine without any written lockout/tagout procedures.

29 CFR  1910.212(a): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  218 W. Main Street, Jackson, MO 63755, Hammer Building: Employer is exposing employees to amputation hazards where the employees are permitted to operate a machine without a guard.  Employees were exposed to amputation hazards where they operated a wooden handle inserting machine without a guard.  NLC, INC-Lenco was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.212(a)(1) , which was contained in OSHA inspection number 1410525, citation number 1, item number 1 and was affirmed as a final order on 09/18/2019, with respect to a workplace located at 319 W. Main Street, Jackson, Missouri 63755.
