The employee was working from the 2nd step of an 8 foot A-frame ladder and the employer did not ensure that it was used on a level surface.

29 CFR 1926.1060(a): The employer did not provide a training program for each employee using ladders and stairways, as necessary, which would enable each employee to recognize hazards related to ladders and stairways and train each employee in the procedures to be followed to minimize these hazards. On or about June 11, 2021, an employee was exposed to a fall of approximately 6 feet while performing painting activities on a new commercial school located at 1157 Ridge, Hinckley, Ohio 44233. The employee was working from the 2nd step of an 8 foot A-frame ladder and the employer did not ensure that the employee had been trained to recognize hazards related to the ladder and how to minimize them.
