29 CFR 1910.217(c)(1)(i): The employer did not provide and ensure the usage of point of operation guards or properly applied point of operation devices on every operation performed on mechanical power press(es) to prevent the operator's hands from reaching the point of operation. See Table O-10: 821 Main St, Holyoke, MA: On June 11, 2021 and at times prior, employees who operated the Bobcat 120 joiner mechanical power press were exposed to crushed by hazards due the lack of point of operation guarding and devices to prevent the operator from being in the danger area while the machine cycled. On June 11, 2021, an employee received a fingertip amputation injury when the machine cycled while the employee's hand was in the point of operation.

29 CFR 1910.147(c)(6)(i):The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirement of this standard were being followed: 821 Main St, Holyoke, MA: On June 15, 2021 and for many months prior, the employer had not performed a periodic inspection that ensured each procedure was inspected for deficiencies during an actual energy control task. Additionally, the employer did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under each energy control procedure they were authorized to use.
