(a) Facility- 2551 Champion Drive, Claysburg, PA: On or about June 11, 2021, an employee operating a UNI Carriers forklift to lift, travel, and dump a two yard dumpster, lost his load when it slid off the forks of the forklift into the compactor.

29 CFR 1904.41(a)(2): 29 CFR 1904.41(a)(2) Annual electronic submission of OSHA Form 300A Summary of Work-Related Injuries and Illnesses by establishments with 20 or more employees but fewer than 250 employees in designated industries.
