29 CFR 1926.651(h)(1):Employees were working in excavations in which there was accumulated water, or excavations in which water was accumulating, and adequate precautions had not been taken to protect employees against the hazards posed by water accumulation. On or about June 15, 2021, an employee was exposed to a cave in while performing sewer line replacement at a residential home located at 10836 Ridge, North Royalton, Ohio 44133. The employee was working in the trench at a depth of approximately 6 feet that had accumulated water and the employer did not ensure to take precautions to remove the water.

On or about June 15, 2021, an employee was exposed to a cave in while performing sewer line replacement at a residential home located at 10836 Ridge, North Royalton, Ohio 44133. The employee was working in the trench at a depth of approximately 6 feet and the employer did not ensure that he was using cave in protection.
