29 CFR  1910.119(i)(1): The employer did not perform a pre-startup safety review for new facilities and for modified facilities when the modification was significant enough to require a change in the process safety information:   (a) On or about 6/8/2021 - Ammunition Manufacturing Building, employees were exposed to deflagration and fire hazards due to manufacturing .30-06 ammunition with explosives such as, but not limited to, NC Powders N100 and N300-series and Ball Powder Propellant and a prestart up safety review had not been conducted to ensure that construction and equipment was in accordance with design specifications; safety, operating, maintenance, and emergency procedures were in place and adequate; a process hazard analysis had been performed and recommendations had been resolved or implemented before startup; and the training of each employee involved in operating a process had been completed.
