29 CFR 1926.102(a)(1):The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation. a) Ludin Amador, at a jobsite located at 50 Coughlin Drive, Delaware, Ohio 43015: On and/or preceding June 14, 2021, the employer failed to ensure employees wore safety glasses while utilizing power tools that presented struck-by hazards to the eyes and face. Employees were using a pneumatic nail guns while installing roofing materials. The employees were not utilizing eye protection, thereby exposing employees to struck-by hazards to the eyes.

Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels shall be protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. a) Ludin Amador, at a jobsite located at 50 Coughlin Drive, Delaware, Ohio 43015: On and/or preceding June 14, 2021, the employer did not ensure employees were protected from falls while engaged in roofing activities. Employees were installing roofing materials without any means of fall protection, thereby being exposed to an 18 foot fall hazard. b) Ludin Amador, at a jobsite located at 50 Coughlin Drive, Delaware, Ohio 43015: On and/or preceding June 14, 2021, the employer did not ensure employees were adequately protected from falls while engaged in roofing activities. Employees were installing roofing materials while using non-safety rated fall protection equipment, thereby being exposed to an 18 foot fall hazard.

Employees were installing roofing materials to the roof of a commercial building. The employees had not been trained on fall hazards or the correct procedures to be followed to minimize fall hazards, thereby exposed to fall hazards of up to 18 feet.
