Employees, without fall protection training, were conducting roofing activities while working on the roof of a new construction house, thereby exposed to 15 foot fall hazards.

An employee was installing roofing materials while using non-safety rated fall protection equipment, thereby exposed to a 15 foot fall hazard.

Employees used ropes, which were tied directly around the trusses at the top nail plate, for positioning while working on the roof of a new construction house, thereby being exposed to a fall hazard of 15 feet.

29 CFR 1926.1053(b)(1):When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. a) Edwardo Lopez Hernandez, at a jobsite located at 5959 Sycamore Trail Drive, Lot 2235, Galena, Ohio 43021 : On and/or preceding June 16, 2021, the employer did not ensure portable ladders that were used for access to an upper landing surface were extended at least 3 feet (.9 m) above the upper landing surface. Employees were using an extension ladder that did not extend at least 3 feet above the edge of the roof, thereby exposed to fall hazards of up to 15 feet.
