Abatement certification must be provided for this item.

29 CFR 1910.134(e)(7)(ii): Additional medical evaluations that complied with the requirements of 29 CFR 1910.134 were not provided when a PLHCP, supervisor, or the respirator program administrator informed the employer that an employee needed to be reevaluated: a) Throughout the site at Porous Materials, Dutch Mill Road, Ithaca, NY, on or about June 17, 2021: Employees of Rader Environmental Services Inc. engaged in mercury remediation of a porosimeter lab and wearing half face air purifying respirators were not provided with annual medical evaluations for respirator use as per the reviewing physician. The original evaluation was performed in 2018 with 3 years elapsing since any further evaluations.
