29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about June 17, 2021 an employee performing roofing operations on a steep sloped commercial roof, was not protected from a fall, exposing the employee to 8 foot fall.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.502(d)(17):The attachment point of the body harness was not located in the center of the wearer's back near shoulder level, or above the wearer's head:  On or about June 17, 2021 an employee's fall arrest harness attachment point was around his waist, exposing the employee to the fall hazards associated with improper use of a fall protection system.  Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR  1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about June 17, 2021 employees performing roofing activities were not trained to recognize and minimize the hazards associated with a fall, exposing the employees to fall hazards up to 25 feet. 29 CFR  1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:(a)   On or about June 17, 2021 employees used an extension ladder that did not extend three feet past the roof they accessed, exposing employees to a 25 foot fall.
