29 CFR  1910.28(b)(1)(i):  Except as provided elsewhere in this section 29 CFR 1910.28, the employer had not ensured that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the systems described in 29 CFR 1910.28(b)(1)(i):  On or about June 17, 2021, employees were exposed to fall hazards of 5'-3" to the ground below while standing on the Bin 5 platform and the employer did not ensure the guardrail system was installed on the ladder side of the platform.

29 CFR  1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief was provided that would prevent pull from being directly transmitted to joints or terminal screws:  On or about June 17, 2021, employees were exposed to electrical hazards located at East Bin 5 when the male receptacle connecting the portable auger to the power supply had exposed inner wires and the employer had not equipped the flexible cord with a device to provide strain relief.
