Location: Jobsite Instance #1 : On 6/10/2021 an employee fell over 14 feet to the ground below and sustained injuries while working from a scaffold platform where fall protection had not been used. Instance #2 : On 6/18/2021 an employee was exposed to falls over 11 feet to the ground below from a scaffold platform while working where fall protection had not been used.

Location: Jobsite An employee was exposed to falls over 11 feet to the ground below and another employee fell over 14 feet to the ground below while working from a ladder jack scaffold platform and sustained serious injuries where fall protection training had not been provided.
