29 CFR  1926.451(g)(1):Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level.  Location: Jobsite  Instance #1 : On 6/10/2021 an employee fell over 14 feet to the ground below and sustained injuries while working from a scaffold platform where fall protection had not been used.  Instance #2 : On 6/18/2021 an employee was exposed to falls over 11 feet to the ground below from a scaffold platform while working where fall protection had not been used.

29 CFR  1926.503(a)(1):The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards.  Location: Jobsite  An employee was exposed to falls over 11 feet to the ground below and another employee fell over 14 feet to the ground below while working from a ladder jack scaffold platform and sustained serious injuries where fall protection training had not been provided.
