29 CFR  1910.151(c): Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:   a) Warehouse: On or about 5/28/2021, the employer exposed employees to chemical hazards, in that they load, unload and store hazardous chemicals such as muriatic acid and there are no facilities for drenching and flushing of the eyes and body in the event of an emergency.

29 CFR  1910.178(l)(2)(ii):The employer did not ensure that each operator had successfully completed the training consisting of a combination of formal instruction (e.g., lecture, discussion, interactive computer learning, video tape, written material), practical training (demonstrations performed by the trainer and practical exercises performed by the trainee), and evaluation of the operator's performance in the workplace.  Warehouse(s): On or about 5/27/2021, and times prior, the employer exposed employees to struck by hazards, in that an employee was operating a Toyota LP Model 8FGU25 S/N 12483 powered industrial truck to move product in and out of the warehouse without have been provided practical training in unloading product from trucks or a demonstration of competency in operating the powered industrial truck.

29 CFR  1910.1200(e)(1):The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met:  a) Warehouse : On or about 5/28/2021, the employees were exposed to chemical hazards in that a written hazard communication program was not implemented  when employees were handling hazardous chemicals such as but not limited to propane and muriatic acid when performing  tasks such as transporting chemical products and switching out propane tanks.
