On or about June 14, 2021, An employee was exposed to fall hazards on a commercial construction site in Decatur, Illinois, and adequate fall protection was not being used around walking/working surfaces with an unprotected side or edge, 6 feet or more above a lower level.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written certification record containing the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employee: On or about June 14, 2021, Employees were exposed to a fall hazard of approximately 14 feet, on a construction site in Decatur, Illinois, and the employer did not maintain evidence of the required training for each employee who might be exposed to fall hazards. There was no written documentation that included the employee's name, date of the training, and the signature of the person who conducted the training or the signature of the employee.
