29 CFR 1910.151(c):Where employees were exposed to injurious corrosive materials, suitable facilities for quick drenching or flushing of the eyes and body were not provided within the work area for immediate emergency use:  On or about June 17, 2021 employees working in the 10 building were exposed to injurious corrosive materials and the employer did not ensure there was a suitable facility to flush the eyes and body for immediate use. a.	Eye wash located on the 10th floor next to the caustic tank had not been inspected and was not available for immediate use due to the lack of water pressure; b.	Eye wash located on the 10th floor next to the DE Room was not provided with an adequate activation handle to provide immediate flushing of eyes; c.	Eye wash located on the 9th floor was not available for immediate use when changing filters due to the lack of water pressure.

29 CFR 1910.22(d)(1):(1) Walking-working surfaces are inspected, regularly and as necessary, and maintained in a safe condition;  On or about June 17, 2021 employees working on the one and a half floor of 17 building were exposed to bodily injuries because the employer did not ensure all walking-working surfaces are maintained in a safe condition. a.	The walkways and working areas of the 1 and a half floor have coatings that have degraded and are not maintained in a safe condition. b.	In the office of the one and a half floor employees are exposed to stepping into a floor hole located in the hallway in front of a refrigerator.
