OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck-by, caught-in, and crushed-by hazards from operating equipment with missing safety equipment: Jobsite: An employee suffered a serious leg injury resulting in amputation when a 1998 "Custom Built" utility trailer (VIN: 1KX29186XW1003018) used for transporting paving equipment that included a LeeBoy 1000D paving machine detached from the Buyer's Products Company PH45 Pintle Hook mounted on a 1998 Peterbuilt dump truck (VIN: 1NPALU0X8WN460575) where the pawl lock clip was missing from the pintle hook .
