29 CFR 1926.502(d)(16)(iii): Personal fall arrest systems, when stopping a fall, were not rigged such that an employee could not free fall more than 6 feet (1.8 m) nor contact any lower level: 1) On or about June 15, 2021, and at times prior, the employer did not ensure that employees were protected from falls, in that personal fall arrest systems were rigged such that an employee could free fall more than 6 feet and could contact the lower level. An employee using a retractable lanyard fell and struck the ground. National Coatings, Inc., exposed employees to fall and struck by hazards.
