29 CFR 1910.28(b)(3)(iv): (iv) Each employee is protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used; (a) On or about June, 21, 2021, in the Main Building on the north side of Station 11, the ladderway access point onto the mezzanine platform did not have a self-closing gate. (b) On or about June, 21,2021, in the Main Building on the north side of Station 13, the ladderway access point onto the mezzanine platform did not have a self-closing gate. (c) On or about June 21, 2021, in the Main Building on the north side of Station 16, the ladderway access point onto the mezzanine platform did not have a self-closing gate.

29 CFR 1910.215(a)(4):Work rests. On offhand grinding machines, work rests shall be used to support the work. They shall be of rigid construction and designed to be adjustable to compensate for wheel wear. Work rests shall be kept adjusted closely to the wheel with a maximum opening of one-eighth inch to prevent the work from being jammed between the wheel and the rest, which may cause wheel breakage. The work rest shall be securely clamped after each adjustment. The adjustment shall not be made with the wheel in motion. On or about 6/21/21, in the Maintenance Shop, employees used a Ryobi 6" grinder which lacked a work rest.
