29 CFR  1926.453(b)(2)(v): A body belt was not worn and a lanyard was not attached to the boom or basket when working from an aerial lift:   a) Jobsite, 4201 Grand Avenue, Neville Island, PA: On or about June 16, 2021, during roofing activities, the employer did not ensure that appropriate fall protection systems/devices were in use while in an aerial lift, exposing employees to fall hazards of up to approximately 30 feet.   Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e).

29 CFR  1926.501(b)(10): Except as otherwise provided in paragraph (b) of this section, each employee engaged in roofing activities on low-slope roofs, with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system, or on roofs 50-feet (15.25 m) or less in width (see Appendix A to subpart M of this part), the use of a safety monitoring system alone [i.e. without the warning line system]:   a) Jobsite, 4201 Grand Avenue, Neville Island, PA: On or about June 17, 2021, during roofing work, the employer did not ensure that appropriate fall protection systems/devices were in use while working on the rear roof section of the building, exposing employees to a fall hazard of approximately 30 feet.
