29 CFR  1910.28(b)(3)(iv): The employer did not provide that each employee be protected from falling into a ladderway floor hole or ladderway platform hole by a guardrail system and toeboards erected on all exposed sides, except at the entrance to the hole, where a self-closing gate or an offset must be used.  On or about June 21, 2021, employees were exposed to fall hazards of approximately 8 feet when entering the Six Pack East Entry Room and the employer did not ensure a guardrail protected the ladderway floor hole.

29 CFR  1910.272(g)(6): Employees at the grain handling facility entered bins, silos, or tanks underneath a bridging condition, or where a buildup of grain products on the side could fall and bury them:  On or about January 13, 2021, employees were exposed to engulfment hazards when entering Bin #6 to remove crusted grain from the wall with scrapers and the employer did not use alternate methods to protect employees from engulfment hazards.
