CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. WORKSHOP: On or about June 2, 2021, the employer had not repaired and/or taken an elevator out of service when the rod/locking system was damaged, allowing the doors to open when the elevator cart was not present on the first floor. This resulted in an employee falling into the elevator shaft from the first floor down to the basement level and sustaining multiple injuries. ABATED DURING THE INSPECTION.
