29 CFR 1926.416(a)(1):Employees were permitted to work in proximity to electric power circuits and were not protected against electric shock by de-energizing and grounding the circuits or effectively guarding the circuits by insulation or other means: (a) Duquesne City School- 300 Kennedy Avenue, Duquesne, PA: On or about June 21, 2021, employees working to remove 277v light fixtures did not de-energize all electrical power circuits in the area of work; an employee was electrically shocked, and injured when he came in contact with the branch circuit wiring in the ceiling.
