On or about June 22, 2021, at the USDA Farm Service Agency, Lares Field Office; employees were exposed to fall hazards. The hazard was created by a sudden change in elevation behind the emergency exit door into the emergency egress route.  b. On or about June 22, 2021, at the USDA Farm Service Agency, Lares Field Office; employees were exposed to fall on same level, trip or slip to the walking surface. The hazard was created by existing moist silty slime, deposited along the emergency egress route surface, behind the exit door.  NOTE: Certification and documentation of abatement must be submitted for all instances of this notice item as required by 29 CFR 1903.19(c) and (d), respectively.
