29 CFR 1910.22(a)(1): (1) All places of employment, passageways, storerooms, service rooms, and walking-working surfaces are kept in a clean, orderly, and sanitary condition. (a) On or about June 23, 2021, the shipping/receiving area around the propane storage for powered industrial truck fuel was cluttered with a metal drum full of boards used as runners under pallets, 2 plastic buckets, a pallet of sidewalk salt, and stacks of sheared metal staged for shipping, which presented trip and fall hazards for employees who needed to access fuel tanks on their powered industrial trucks.

29 CFR 1910.303(g)(2)(ii): In locations where electric equipment is likely to be exposed to physical damage, enclosures or guards shall be so arranged and of such strength as to prevent such damage. a) On or about June 23, 2021, the electric breaker box located within ten feet of the propane storage rack was missing a knockout on the left side near the propane storage and the cover for the panel circuit breakers was sitting on top of the box.

29 CFR 1904.4(a):Basic requirement. Each employer required by this Part to keep records of fatalities, injuries, and illnesses must record each fatality, injury and illness that is (a) work related; (b) a new case; and (c) meets one or more of the general recording criteria of 29 CFR 1904.7 or the application to specific cases of 29 CFR 1904.8 through 1904.12. On or about June 24, 2021, the employer did not record the following workplace injury on the OSHA 300 Log for the calendar year 2021. (a) On or about April 29, 2021, a recordable injury occurred when an employee slipped and fell on a wet floor, suffering a closed, nondisplaced fracture of the left hand.
