OSH ACT of 1970 Section (5)(a)(1): Section 5(a)(1) of the Occupational Safety and Health Act of 1970: The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees from hazards associated with exposure to anhydrous ammonia: a) Jobsite, 602 Thomas Road, Monroeville, PA 15146: On or about May 10, 2021, the ammonia piping systems were not marked properly. There were no legends on the ammonia piping systems that were located on the outside of the building and some legends on the ammonia piping systems inside the building were deteriorated and incomplete.

29 CFR 1910.147(f)(2)(i): When outside servicing personnel were engaged in activities in a facility, the onsite employer and the outside employer did not inform each other of their respective lockout or tagout procedures: (a) Jobsite, 602 Thomas Road, Monroeville, PA 15146: On or about May 10, 2021, when outside servicing personnel (employees of R.C.E. Heat Exchangers, LLC) were engaged in maintenance activities on compressor coolers at this facility, the outside employer and the onsite employer (Montauck Energy Holdings, LLC) did not inform each other of their respective lockout or tagout procedures prior to the start of maintenance work.
