29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  521 S. Shore Drive, Lake Winnebago, Missouri- Employees were exposed to overhead hazards from elevated materials and equipment and were not protected from impact by the use of any means of head protection.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:   (a). Worksite located at 521 S. Shore Drive, Lake Winnebago- Employees were observed framing up the 2nd level and were not protected from falling by the use of any means of fall protection.   (b). Worksite located at 521 S. Shore Drive, Lake Winnebago- Employees were exposed to a fall hazard on the 1st level in that the stairway opening was left exposed for employee to fall to the basement level.
