29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6): On or about June 25, 2021, the employer failed to ensure that when employees perform the task of lock out tag out prior to servicing or repairing machinery or equipment such as but not limited to the conveyor lines, that the steps identified in this standard are fully detailed and follow sequentially prior to starting the service or repair.
