29 CFR 1910.22(a)(3): The employer did not ensure that walking-working surfaces are maintained free of hazards such as sharp or protruding objects, loose boards, corrosion, leaks, spills, snow, and ice: a) Hessian Company dba Faddis Concrete Products, Flatbed trailer #24-310, on or about June 25th, 2021 and times prior thereto: Employees that accessed flatbed trailer #24-310 were exposed to trip and laceration hazards while walking/working on top of the trailer. The walking working surface of the flatbed trailer was damaged in that, wooden boards were deteriorated/missing in various areas exposing open areas on the walking/working surface. Additionally, employees were exposed to potential laceration hazards associated with various metal parts that were exposed from the metal frame of the flatbed truck. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.

29 CFR 1910.22(d)(2): The employer did not ensure that hazardous conditions on walking-working surfaces are corrected or repaired before an employee uses the walking-working surface again. The employer did not ensure that if the correction or repair cannot be made immediately, the hazard is guarded to prevent employees from using the walking-working surface until the hazard is corrected or repaired. a) Hessian Company dba Faddis Concrete Products, Flatbed trailer #24-310, on or about June 25th, 2021 and times prior thereto: Employees that accessed flatbed trailer #24-310 were exposed to trip and laceration hazards while walking/working on top of the trailer. Hessian Company dba Faddis Concrete Products, Flatbed trailer #24-310, on or about June 17th, 2021 and times prior thereto: The storage of metal wire mesh panels on the trailer created a hazard to employees while they were unloading the panels. After releasing the securing ratchet strap, an employee was struck by falling panels. ABATEMENT CERTIFICATION REQUIRED: Abatement certification is required within 10 days after the abatement date noted on the citation (see below). This certification shall include a statement that abatement is complete, the date abatement was completed, method of abatement, and state that employees and their representatives were informed of this abatement.
