29 CFR  1903.19(d)(1):The employer did not submit to the Agency documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required.  (a)  The employer failed to provide documentation to the Agency demonstrating that abatement is completed for the serious violations that were issued by the Agency.

29 CFR  1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a)  On or about June 24, 2021, employees engaged in roofing a new residential property on a 9/12 pitched roof, were not protected from falling to the lower level. This exposed employees to a fall hazard of approximately 11 feet.  The L&M Contractors LLC, company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1531490, citation number 1, item number 1, and was affirmed as a final order on June 16, 2021, with respect to a workplace located  at W139 N6323 Bridal Wreath Lane, Menomonee Falls, WI 53051.  The L&M Contractors LLC, company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1467083, citation number 1, item number 1, and was affirmed as a final order on June 1, 2020, with respect to a workplace located  at 3726 Princeton Way, Sturtevant, WI 53177.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  (a)  On or about June 24, 2021, the employer did not ensure that employees exposed to potential fall hazards had been trained to recognize and minimize the fall hazards associated with their work.  The L&M Contractors LLC, company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 1531490, citation number 1, item number 2, and was affirmed as a final order on June 16, 2021, with respect to a workplace located  at W139 N6323 Bridal Wreath Lane, Menomonee Falls, WI 53051.
