29 CFR 1910.29(b)(2) (2) Midrails, screens, mesh, intermediate vertical members, solid panels, or equivalent intermediate members are installed between the walking-working surface and the top edge of the guardrail system as follows when there is not a wall or parapet that is at least 21 inches (53 cm) high:  (a) On or about May 24, 2021, at the worksite located at 1205 65th St., Kenosha, WI, the employer did not ensure adequate fall protection was utilized on the top of  the Automa 24 machine, thereby exposing workers to fall hazards.   (b) On or about May 24, 2021, at the worksite located at 1205 65th St., Kenosha, WI, the employer did not ensure adequate fall protection was utilized on the top of the Automa 25 machine, thereby exposing workers to fall hazards.

29 CFR  1910.212(a)(1):One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:  (a) On or about June 23, 2021, at the worksite located at 1205 65th St., Kenosha, WI , the employer did not ensure adequate machine guarding was installed on the Automa 25 machine. The machine had a damaged section that rendered the safety interlock inoperable, thus, exposing workers to struck-by and caught-in-between hazards when the access door was in the open position.
