29 CFR 1926.501(b)(1):Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the south east side of the construction site, employees working on a parking lot ramp installing wire mesh form to rebars preparing for concrete pour work without fall protection were expose to fall hazards of approximately 20 feet to the ground.

On the south east side of the construction site, employees engaged in preparing concrete wooden forms utilized an extension cord, to energize a skill saw, with the outer sheathing split exposing them to electrical shock hazards.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards. On the construction site, the employer did not develop or provide fall protection training to the employees engaged in laying metal mesh and attaching it to rebar on an elevated ramp and how to recognize and eliminate hazards such as fall hazards.
